Japan, South Korea, China, Mongolia, and North Korea. My video above explores how many Americans entered each country over the last year, as well as over their lifetimes, as well as the total percentage who would have visited each country over their lives, as well as the intrepid (defined as stupid) who would have visited all five!! 
I personally have visited 4/5, with North Korea, as for the rest of the planet, remaining a mystery to my eyes.
